Pet-Form Chewable Vitamin-Mineral for Dogs and Cats

Pet-Form is a nutritional support supplement for dogs and cats. It includes 11 vitamins and 8 minerals with the addition of Zinc. It also includes a source of protein and unsaturated fatty acids. Pet-Form is designed to supplement a regular diet and comes with a tasty meat flavored tablet. Available in 50, 150, and 500 count bottles.

 

Ingredients: Brewers dried yeast, dicalcium phosphate, lactose, microcrystalline cellulose, calcium carbonate, glycerin, silicon dioxide, choline chloride, DL-methionine, magnesium stearate, ferrous sulfate, niacin, dextrose, fish oil, vitamin A palmitate, vitamin E supplement, thiamine mononitrate, manganese sulfate, zinc oxide, vitamin B12 supplement, copper sulfate, polyvinylpyrrolidone, potassium phosphate, riboflavin, calcium pantothenate, vitamin D3 supplement, cobalt sulfate, folic acid, ethylenediamine dihydroiodide, pyridoxine HCL. Artificial colors: Yellow 6 Lake, Red 40 Lake, Blue 1 Lake.

Feeding Directions: Feed from the hand or crumble and mix into the food.

Dogs: Small breeds, 1/2 to 1 tablet. Medium breeds, 1 tablet. Large breeds, 2 to 4 tablets.

Cats: 1 tablet (All breeds and sizes).

Store at room temperature. Protect from light. Avoid excessive heat (40° C or 104° F).
