Is there a recommended minimum weight for this medication? My 15 lb chiweenie has been on regular Levetiracetam (and Phenobarbital) for about two years now, with doses given every 8 hours. My schedule is changing soon, and administering twice a day would be SO much easier.?

Levetiracetam is an anti-seizure medication, often prescribed extra-label for canine epilepsy. Transitioning from immediate-release (IR) formulation to extended-release (ER) formulation can be done. However, this requires careful monitoring of clinical response and the consideration of possible dose adjustments from your veterinarian due to the differences in the rate or extent of absorption of the drug. According to Plumb’s Veterinary Handbook, dosing using ER tablets begins with 30 mg/kg by mouth with or without food every 12 hours. Because of the drug’s safety, doses may be increased (up to 60 mg/kg by mouth every 12 hours) further until efficacy is achieved, adverse effects become apparent, or drug cost becomes prohibitive. Administering every 24 hours may be acceptable in some dogs. Higher doses may be needed if using concurrently with phenobarbital. The smallest extended-release tablet is 500 mg, so it may not be practically administered to small dogs. It is recommended to consult with your veterinarian about whether the extended-release formulation is a good fit for your dog. They will be able to best address your questions and concerns as they are most aware of your dog’s condition and needs.

My dog has been taking Levetiracetam extended release 500mg every 12 hours (10a & 10p) for a few months and I was told by my primary veterinarian that it's important to give it at these exact times. He is scheduled for his annual dental cleaning with another veterinarian in the practice (my veterinarian is on maternity leave) and he said it was okay to give it to him before I drop him off at 8a for the cleaning. Should I be concerned with the dosage time change and him being put under anesthesia? Thank you?
